All faculty members, including lecturers, are reminded to accomplish and sign their Faculty Service Records (FSRs) for First Sem AY 2026–2027. Faculty members should also submit copies of their approved ALC, RLC, ELC, and SLC requests, if applicable.

Action Required:

• Accomplish and sign your FSRs through your CRS account.
• Submit approved ALC/RLC/ELC requests, when applicable, to the designated office.
• UP CA admin to consolidate and submit the documents to OVCAA.
